ASSESSMENT SCREENING TOOL FOR USE WITH
CANCER, PALLIATIVE CARE & OTHER MEDICAL ILLNESSES
This screening tool is available for any medical center to download and use with expressed permission from the copyright owners. We are promoting this crucial endeavor in whole patient-centered care, for all centers to be at leading edge with the standards of care. You are the Champions that will make this necessary component of care possible, allow your Cancer and Medical Centers to fully comply with national standards, and go beyond, aligning yourselves with the leading Centers in the U.S. We had a great team behind us in developing this instrument; and made every attempt to find a balance between the needs of clincial staff, patients and their families. Our ongoing research from our team, including City of Hope's Nationally Funded Research Project and trainings, collaborated in the structure of this assessment tool, with Dr. William Breitbart, MD, Chairman Department of Psychiatry and Behavioral Sciences, from Memorial Sloan Kettering Cancer Center in NYC. His research and pioneering work for the last 30 years in psycho-social oncology treatment, psychosomatic medicine, and palliative care, including being part of designing the Distress Thermometer assessment and several other assessment tools, as well as an esteemed researcher and author, editor-in-chief of Palliative and Supportive Care, and founding member of APOS and IPOS, has informed this assessment screening, along with its unique design and layout, making it stand out from current assessment tools in the industry. It is currently being used and pilot studies are underway at JFK Comprehensive Cancer Institute/ JFK Medical Center in Atlantis, FL, a teaching affiliate of the University of Miami Miller School of Medicine, as well as other locations.
